Information Needed for Debtor/Creditor Counseling

Information for all creditors owed (whether or not you intend to pay them), including:
- Name and address
- Account #'s
- Amount owed and what the debt was for - what goods or services were received
- Date the debt began to accrue
If the debt has gone to collection, and/or been sold to another agency and/or now involves a lawyer, we need the names and addresses of all parties involved.
Information and available paperwork on any type of lawsuit in the past two years you were a party to or are currently a party to. This could be for debt collection, a civil law suit such as a personal injury action, any criminal proceeding, or anything filed with a court.
If necessary, we can obtain your credit report for you. Fee: $35 single / $50 joint.

Asset information:
- What do you own? How much is it worth? Who has a lien on these things?
- If you own your house(s), when did you buy it? We also need the legal description(s) of your house(s) (found on your abstract(s).)
Monthly Expense Information: What are your monthly expenditures? We have a packet you will receive asking for this information. Please fill it out as detailed as possible.
Copy of your tax returns for the last two years immediately preceding the bankruptcy filing. If you have not yet filed for either of the previous two years please see your tax professional or accountant to have this done as soon as possible.
Wage information for the last six months, including a breakdown of the deductions for each pay period.
A wage statement from your employer showing any wages owed to you as of the date of your bankruptcy filing ("i.e.: if your employment had been terminated on the bankruptcy filing date, how much in wages, including any hold-back, would have been due and owing to you from your employer?").
Your total gross income for this year, to date.
